Background: Assessment of oral epithelial dysplasia is the gold standard for investigating the risk of malignant progression. The World Health Organization (WHO) methods and the binary system have limitations. This study assess the inter- and intra-observer variability of the architectural and cytological criteria and the classification of the presence and degree of epithelial dysplasia in oral lichen planus (OLP) and oral lichenoid lesion (OLL), using both the 2017 WHO criteria and the binary system. Material and Methods: The sample consisted of 65 biopsies from lesions classified as OLP and OLL according to the American Academy of Oral and Maxillofacial Pathology (AAOMP) criteria. The histological slides were reevaluated by two oral pathologists. Results: The individual alterations with most inter-observer disagreement were atypical mitotic figures (43.1%), loss of cohesion between epithelial cells (38.5%) and drop shape rete ridges ridges (38.5%). Inter-observer agreement analysis did not show statistically significant agreement regarding the classification of epithelial dysplasia grade by WHO criteria, only regarding the binary system classification (k=0.257; p=0.035). Intra-observer agreement analysis by evaluator 1 showed that the classification of epithelial dysplasia grade according to both methods had statistically significant agreement (k=0.546; p=0.004, k=0.861; p<0.001). Considering evaluator 2, only the evaluation of the WHO system classification showed statistically significant agreement (k=0.593; p=0.010). Conclusions: The evaluation of epithelial dysplasia is subjective and focal changes and inflammatory infiltrate, characteristic of OLP and OLL, can increase the degree of disagreement among evaluators. The binary system presents better inter-observer agreement, while the WHO system presents better intra-observer agreement. Cheilitis is a term given to the inflammation that occurs in the vermillion of the lips. The exfoliative type is an uncommon form of cheilitis, which is characterized by inflammation and desquamation of the lip. It can cause aesthetic problems and compromise daily eating and phonation. The aim of this paper is to describe two cases of exfoliative cheilitis in young persons under periods of emotional stress and parafunctional habits. A 22-year-old white male and an 18-year-old black female presenting edema, intense dryness, and slight desquamation on the vermilion of the lips. In the second case, fissures with bleeding were also observed. Oral lesions were associated with intense emotional stress. The diagnosis of both was made based on the clinical presentation and the exclusion of other conditions. Although the patients have presented a significant improvement after the corticosteroid treatment, they still have a recurrence in stressful episodes. Detailed clinical examination and complementary exams are fundamental for determining associated factors and correctly diagnosing exfoliative cheilitis. OBJECTIVE: To evaluate the prevalence and clinical aspects of oral candidiasis in patients hospitalised in the intensive care unit. METHODS: This is a longitudinal and prospective study that included 48 participants hospitalised in the intensive care unit. Sociodemographic data, presence of systemic disorders, use of medications, laboratory tests, cause of hospital admission, type of breathing, and length of hospital stay were obtained from medical records. Oral clinical inspection and cytopathological examinations were performed on all participants. The diagnosis of clinical candidiasis was based on the presence of clinical alterations together with positive cytopathological examination results. The diagnosis of subclinical candidiasis was based on the absence of clinical lesions and a positive cytopathological examination. The absence of oral candidiasis was considered when the participant did not present oral lesions and had a negative cytopathological examination. RESULTS: Clinical candidiasis was present in 18.8% of the 48 participants, and 45.8% of them had the subclinical form. Levels of urea (P = 0.005), creatinine (P = 0.009), haemoglobin (P = 0.009), haematocrit (P = 0.011), bands (P = 0.024), international normalised ratio (INR; P = 0.034), types of breathing (P = 0.017), length of hospital stay (P = 0.037), and outcome (P = 0.014) demonstrated statistically significant differences between the groups with and without oral candidiasis. CONCLUSIONS: Clinical and subclinical forms of oral candidiasis are frequent in intensive care unit patients. Levels of urea, creatinine, haemoglobin, haematocrit, bands, INR, type of breathing, length of hospital stay, and outcome can be associated with the presence of candidiasis.

BACKGROUND: It is difficult to distinguish the clinical and histopathological aspects of oral lichen planus lesions from those of oral lichenoid reaction. Some criteria were proposed to distinguish them, mainly because they have different biological behaviors. The aim of the present study was to compare the lymphocyte population and the expression of E-selectin between these lesions. METHODS: Participants with a clinical diagnosis of oral lichen planus (GOLP) and oral lichenoid reaction (GOLR) who needed to perform a biopsy were selected. The tissue was frozen and immunostaining was performed for CD3/CD4, CD3/CD8, CD4/CLA, CD8/CLA, and CD62E. The analysis of each immunostaining was accomplished using the ImageJ program. RESULTS: In total, 25 participants with oral lichen planus and 11 with oral lichenoid reaction were seen. In the evaluation of CD3 + CD4+/CD3 + and CD3 + CD8+/CD3 + proportions, there was a higher percentage of these cells in the oral lichen planus group when compared with the oral lichenoid reaction group (p = 0.027 and p = 0.038 respectively). The average number of CLA + lymphocytes for CD4+/CLA + and CD8+/CLA + in both groups was not statistically significant (p = 0.840; d = 0.363). In GOLP, the number of CD4 + CLA+/E-selectin and CD8 + CLA+/E-selectin was not statistically significant (p = 0.951 and p = 0.454 respectively); neither in GOLR (p = 0.454 and p = 0.989 respectively). CONCLUSION: Our results indicate that CD3 + CD4+, CD3 + CD8+, CD4 + CLA+, CD8 + CLA + lymphocytes and E-selectin are present in both lesions. However, the proportion of CD3 + CD4+/CD3 + and CD3 + CD8/CD3 + cells is higher in the oral lichen planus group when compared with the oral lichenoid reaction group, suggesting that these cells may be important for the etiopathogenic mechanism of these lesions.

BACKGROUND: The diagnosis of oral lichenoid lesions (OLL) remains a challenge for clinicians and pathologists. Although, in many cases, OLL cannot be clinically and histopathologically distinguishable from oral lichen planus (OLP), one important difference between these lesions is that OLL has an identifiable etiological factor, e.g. medication, restorative material, and food allergy. The list of drugs that can cause OLL is extensive and includes anti-inflammatory drugs, anticonvulsants, antihypertensives, antivirals, antibiotics, chemotherapeutics, among others. This work aimed to perform a literature review of OLL related to chemotherapy drugs and to report two cases of possible OLL in patients with B-cell and T-cell non-Hodgkin lymphomas in use of chemotherapy and adjuvant medications. We also discuss the challenge to clinically and histopathologically differentiate OLL and OLP. CASE PRESENTATION: In both cases, oral lesions presented reticular, atrophic, erosive/ulcerated, and plaque patterns. The diagnosis of OLL was initially established in both cases by the association of histopathology and history of onset of lesions after the use of medications. Although the patients have presented a significant improvement in the oral clinical picture for more than 2 years of follow-up, they still have some lesions. CONCLUSION: A well-detailed anamnesis associated with the drug history, temporal relationship of the appearance of the lesions, and follow-up of patients are fundamental for the diagnosis of OLL related to drugs. Nevertheless, its differentiation from OLP is still a challenge.

OBJECTIVE: This study is the first to analyse the prevalence of oral candidiasis in onco-haematological patients by physical and oral cytopathological examinations. METHODS: This is a cross-sectional and observational study with a retrospective sample composed of participants hospitalised in the haematology clinic, who were diagnosed with haematological diseases. All participants received an oral mucosal examination and scraping from oral mucosa. RESULTS: Of the 62 participants, 56.5% were male and 82.3% were white, with mean age of 57 years. Lymphoma was the most common haematological disease (24.2%). In total, 48.4% of the sample was diagnosed with oral candidiasis. Of the participants with oral candidiasis, 13 (21.0%) had a clinical diagnosis. Cytopathological analysis revealed 17 more (27.4%) cases without oral lesions indicative of candidiasis. Erythematous candidiasis (P = 0.02), pseudomembranous candidiasis (P < 0.001), clinical candidiasis (P < 0.001), fibrous hyperplasia (P = 0.032), and coated tongue (P = 0.012) showed a correlation with a candidiasis cytopathological diagnosis. CONCLUSIONS: Oral candidiasis is common among patients with haematological disease, and the cytopathological examination proved to be a useful tool, confirming clinical diagnosis of candidiasis and identifying subclinical cases. These data are of great relevance considering the possible complications that these patients may develop, such as longer hospitalisations, worsening of the general condition or even death due to candidemia.

The most common oral choristomas are consisted of thyroid tissue and bone. The presence of sebaceous glands in the oral mucosa, especially in the buccal mucosa and labial mucosa, is often considered a normal anatomical variation since they are observed in about 80% of the population and are called ectopic sebaceous glands or Fordyce's granules. However, the presence of these glands on the tongue is rare, with only 11 cases in the dorsum of the tongue reported in the English literature, and it is considered a choristoma. This paper aims to report the third case in the literature of a congenital sebaceous choristoma on the tongue. An 11-year-old white male patient presented a firm sessile papule, without color alteration, measuring 0.4 cm x 0.3 cm in diameter, in the middle third of the dorsum of the tongue with a slight increased size in the last months. Histopathological examination showed an invagination of the epithelium into the connective tissue, forming a ductal structure covered by stratified squamous epithelium. The deeper areas had normal well-differentiated sebaceous glands, with ducts connected to the central duct. Considering clinical and histopathological findings the diagnosis was sebaceous choristoma. Despite being rare, sebaceous choristomas should also be considered in the differential diagnosis of tongue abnormalities or lesions. Although the pathogenesis is not well understood, the present report, as a congenital choristoma case in the midline, reinforces the hypothesis of a disorder with embryological origin and a possible relationship with thyroglossal duct remnants. We conducted an integrative review on oral manifestations in patients with COVID-19 based on the current available literature evidence. A bibliographic search was carried out on March 11, 2021, among published studies in the years 2019-2021 in the PubMed database and based on the search strategy ("COVID-19" AND "oral lesions" OR "oral mucositis" OR "oral manifestation"). After applying the inclusion and exclusion criteria, 29 articles were considered suitable for this review. A total of 110 cases of patients with COVID-19 who had oral manifestations were reported. The presence of ulcerated lesions was the most common finding, having a herpetiform and aphthous clinical pattern observed in most cases. Macules, petechiae, hemorrhagic blisters, pustular enanthem, mucositis, and halitosis were also among the most frequently described oral manifestations. The tongue was the most commonly affected site, followed by the palate and lip. Most of the reported cases were diagnosed only by the clinical aspect of the lesion associated with a positive SARS-CoV-2 test or the presence of other COVID-19 symptoms. Current scientific evidence still could not affirm that most of the oral lesions observed in patients with COVID-19 are related to the virus's direct or indirect action on the oral mucosa. To confirm this association, prospective and longitudinal studies are further needed, together with a larger number of patients, complemented by histopathological examination of these lesions. Additionally, molecular techniques, such as immunohistochemistry and in situ hybridization, may be necessary to perform the differential diagnosis with other oral lesions.

ABSTRACT: Lichen planus (LP) is a mucocutaneous immune-mediated disease of unknown etiology. It is more prevalent in women and usually occurs between the third and sixth decades of life. Oral lesions may or may not be associated with skin and genital lesions. Although the role of genetic factors is still undetermined, reports of LP in more than one family member are not uncommon. However, the occurrence of LP in monozygotic twins is rare. We report a rare case of 42-year-old female monozygotic twins presenting oral LP. This report is even rarer because one of the patients had cutaneous lesions of an unusual variant of LP (LP pigmentosus) and the other had an uncommon association with lichen sclerosus. The etiology and pathogenesis of LP are still uncertain. However, despite being rare, its occurrence in family members and monozygotic twins suggests that genetic factors are involved in its development.

Objetivo: Avaliar a associação entre a prevalência de cárie dentária em relação a frequência de consumo de alimentos ricos em açúcar, condições socioeconômicas e escolaridade dos pais de crianças e adultos jovens. Material e Métodos: Aplicou-se questionários socioeconômicos com pais e/ou responsáveis de 36 crianças e adolescentes na faixa etária entre 7 e 20 anos de um clube do Município de Alegre - ES. A cárie dentária foi avaliada através do Índice de Dentes Cariados, Perdidos e Obturados (CPO-D) e a placa bacteriana por meio do Índice de Higiene Oral Simplificado (IOHS). Posteriormente, realizaram-se palestras e atividades educativas de higiene bucal e de educação nutricional. Reavaliou-se então índice de placa bacteriana duas semanas após a intervenção. Para avaliação estatística usou-se regressão logística para testar associações e teste t pareado para índice de placa bacteriana. Resultados: Do total de crianças avaliadas, 67% apresentou história pregressa e atual de cárie, na qual a mediana do CPO-D encontrado foi de 2. Para a avaliação do índice de placa bacteriana, observou-se que a intervenção apresentou efetividade (p<0,05) uma vez que IHOS inicial foi de 1,98 e o final foi de 1,43. Conclusão: Foi encontrada alta prevalência de COP-D polarizado na amostra avaliada, e também detectado que as ações de educação em saúde foram efetivas na redução de placa bacteriana


Objective: to evaluate the association between the prevalence of dental caries, sugary food intake and socioeconomic conditions of children and young adults. Material and methods: socioeconomic questionnaires were applied to parents and/or guardians of 36 children and adolescents attending a soccer club in the city of Alegre ­ ES, Brazil, aged between 7 and 20 years. To assess food intake, a Food Frequency Questionnaire (FFQ) was applied. Tooth decay was assessed using the Decayed, Missing, Filled Teeth Index (DMFT) and plaque through the Simplified Oral Hygiene Index (OHI-S). Subsequently, lectures and oral hygiene and nutritional education activities were held. The plaque index was then reevaluated two weeks after the intervention. The presence of caries was expressed using relative frequency. In order to evaluate the factors associated with dental caries, a logistic regression model was used (p <0.05), and to evaluate the effect of the intervention, the paired t-test was used to compare the plaque index. Results: 67% out of the assessed children had past and current history of decay, with a median DMFT of 2. Among the variables assessed 'family income', 'parents' educational level' and 'consumption of high-sugar foods' no statistically significant association (p <0.05) with the presence of dental caries was observed. To evaluate the bacterial plaque index, it was observed that the intervention was effective (p <0.05) with an initial OHI-S of 1.98 and final of 1.43. Conclusion: a high prevalence of polarized DMFT was found in the sample. The health education actions were effective in reducing plaque. Almost all of the sample was found to have the habit of consuming sugary food four or more times a week. No significant associations were found between dental caries and the variables analyzed.

ABSTRACT Objective: To evaluate the association between the presence of tooth decay and the eating habits, oral hygiene habits, and socioeconomic profile of children in the fifth grade at the state secondary and elementary school Pedro Simão in Alegre, ES. Methods: Questionnaires were used to evaluate the socioeconomic profile, eating habits, and oral hygiene habits of the fifth-grade students. Then an epidemiological survey on dental caries and on the presence of dental plaque was conducted. After the first clinical examination, the students completed educational and preventive activities about nutritional and oral hygiene education. One week after the performance of the educational actions, the dental plate index was evaluated. Results: The variables "family income" and "number of members" in the family were statistically significant (p≤0.20) with caries association using a bivariate regression analysis. However, variables related to diet did not present a significant association with caries (p>0.20). The initial and final IHOS were 1.85 and 0.92, respectively, which was significantly lower after the intervention (p<0.05). The DMFT was 1.81, which is higher than the average for the Southeast Brazil, obtained in 2010 by the Brazilian National Oral Health Survey (SB Brazil). Conclusion The caries prevalence was high in the sample studied, and the intervention was important to reduce the students' dental plaque levels and improve their oral hygiene condition. Thus, it is necessary to develop continuous educational programs so that oral health is maintained.


RESUMO Objetivo: Avaliar a associação entre a presença de cárie dentária com a dieta alimentar, hábitos de higiene oral e o perfil socioeconômico de crianças do 5° ano da Escola Estadual Ensino Fundamental e Médio Pedro Simão do Município de Alegre, Espírito Santos, Brasil. Métodos: Foram aplicados questionários para avaliar o perfil socioeconômico, os hábitos alimentares e hábitos de higiene oral de 26 escolares do 5° ano; em seguida, foram realizados levantamento epidemiológico em relação à cárie dentária e a presença de placa bacteriana. Após o primeiro exame clínico, foram realizadas atividades educativas e preventivas em educação nutricional e de higiene oral. Uma semana após a aplicação das ações educacionais, o índice de placa foi reavaliado. Resultados: As variáveis 'renda familiar' e 'número de membros na família' foram estatisticamente significativas (p≤0.20) pela análise de regressão bivariada, tendo associação com a cárie, porém, as variáveis relacionadas à dieta não apresentaram associação significativa (p>0.20) com a cárie. O IHOS inicial foi 1.85, e o final 0.92, sendo significativamente menor (p<0.05) após a intervenção realizada. O CPO-D foi de 1.81, resultado acima da média da região Sudeste do Brasil, obtida em 2010 pela Pesquisa Nacional de Saúde Bucal (SB Brasil). Conclusão: A prevalência de cárie foi elevada na amostra estudada e a intervenção realizada foi de suma importância para reduzir os níveis de placa bacteriana dental dos escolares e melhorar a condição da higiene oral, afirmando que é necessário desenvolver ações educativas continuadas para que a saúde seja mantida.

Objetivo: Analisar expressão da cadeia γ2 da laminina 332 na membrana basal de vasos de carcinoma de células escamosas orais (CCE) e seu aspecto de continuidade para detecção de possíveis processos invasivos vasculares, importante evento na progressão da doença. Ainda, associar os aspectos clínicos do paciente e da lesão com os achados microscópicos. Material e Métodos: Foi realizada técnica imuno-histoquímica para detecção da cadeia γ2 da laminina 332 em 16 casos de CCE, sendo oito Bem Diferenciado e oito Moderadamente Diferenciado. Os aspectos microscópicos avaliados foram expressão da molécula na membrana basal de vasos e o seu aspecto de continuidade. Possíveis associações entre dados clínicos e achados microscópicos foram avaliadas. Resultados: Um total de 15 casos (93,75%) apresentaram vasos com expressão da cadeia γ2. Em CCE Bem Diferenciados, 85,71% das lesões apresentaram vasos com perda de continuidade da membrana basal, enquanto que em CCE Moderadamente Diferenciados 100% dos casos tiveram vasos com esse aspecto. Em relação às associações entre aspectos clínicos e integridade da membrana basal vascular, nos CCE Bem Diferenciados foram observadas correlações positivas da perda de continuidade com o gênero e idade acima de 60 anos (p < 0,0001). Para CCE Moderadamente Diferenciado, as associações foram com gênero masculino (p < 0,0001), hábito de fumar (p < 0,0001) e idade < 60 anos (p < 0,0001). Conclusão: Nossos dados sugerem que a expressão da cadeia γ2 em estruturas vasculares está presente em lesões de CCE e pode estar associada ao comportamento mais agressivo da doença.


Objective: 1) To analyze the γ2 chain expression of laminin 332 in the basement membrane of vessels of oral squamous cell carcinoma (OSCC), and the appearance of continuity, to identify possible vascular invasive procedures which are important events in the disease progression. 2) To examine the association between clinical aspects from patients and microscopic findings from lesions. Material and Methods: Immunohistochemistry was performed to detect γ2 chain of laminin 332 in 16 cases of OSCC; of which, eight were classified as being "Well Differentiated" and eight as being "Moderately Differentiated." Microscopic aspects were evaluated, including the expression of the molecule in the basement membrane of vessels and the appearance of continuity. Results: 15 cases (93.75%) showed vessels with γ2 chain expression. In OSCC defined as being "Well Differentiated," 85.71% of lesions presented with loss of vessel continuity of the basement membrane, whereas in OSCC defined as being "Moderately Differentiated," 100% of presented vessels with this aspect. Regarding associations between clinical aspects of patients and integrity of the vascular basement membrane, in OSCC defined as being "Well Differentiated," positive correlations were observed between loss of continuity and both gender and age >60 years (p < 0.0001). For OSCC defined as being "Moderately Differentiated," positive associations were observed for being male (p < 0.0001), currently smoking (p < 0.0001) and being <60 years old (p < 0.0001). Conclusion: Our data suggest that the γ2 chain expression in vascular structures is present in OSCC and is associated with more aggressive disease.
